Scheduling an Appointment

When you call for an appointment, the office will get some basic information from you such as address, phone number, and health insurance information and in a word, why you are scheduling. You do not need to provide a reason, if you wish, simply say, “It’s personal” and you may speak only to Dr. Velez about your concerns.
Confidentiality

Confidentiality

Your confidentiality is very important, both to you and to Dr. Velez. Anything you say is kept in strict, professional confidence. The office of Dr. Jane Velez conforms to HIPPA rules and regulations. You can be provider a copy of HIPPA when you come to our office, or we can mail or fax you a copy. HIPPA is designed to protect your privacy. Only a diagnostic code will be provided for billing to your insurance company. No other information will be released unless you request that information be released, and sign a form requesting that release either in person on via fax.

Insurance and/or Payments

Dr. Velez accepts almost all Health Insurance and Employee Assistance Programs (EAP). We will validate the insurance coverage and your co-pay, but you may wish to check with your insurance company to verify your mental health benefits and required co-payments.

We accept Medicare, but due to issues beyond our control, we cannot accept Medicaid at this time. This may change in the future.

We accept MasterCard, Visa, Debit cards and cash.
